MEDICAL CORPORATION OR LIMITED LIABILITY COMPANY CERTIFICATE

  1. Contact Secretary of State and obtain Certificate
  2. Submit to Board office:
    1. Complete the Application found in the MySD Account
    2. copy of Certificate - obtained from Secretary of State
    3. Articles of Incorporation or Organization and Bylaws

NOTE:

  1. Medical Corporation---must state that all officers, directors, and shareholders are licensed pursuant to SDCL 36-4.
  2. Limited Liability Company---It must state that all officers, members, and managers are licensed pursuant to SDCL 36-4.

As a medical professional licensed by this Board, renewal of your South Dakota medical corp. or LLC is entirely your responsibility. Reminders, if sent, are a courtesy. The licensing board is under no obligation to send reminders to renew a professional license or registration.
