It is the responsibility of all licensees to renew their license before expiration. Failure to renew shall result in automatic forfeiture or suspension of the license by statute. What does primary source verification mean?

The definition of Primary Source Verification is the act of obtaining the applicant's credentials (the document itself or verification of the document) directly from the original or primary source. Verification of edcuational programs, exams, and state licenses for the application process all must be received primary source.



What is the expected timeframe for an application?

We cannot provide timelines for the application process. This is because each application is unique and it is not possible to determine the time needed to have the requried verifications returned to the Board office. Once an application is submitted it must be completed within 120 days.

20:78:03:02. Initial review. The executive secretary shall review all license, permit, and certificate applications subject to this chapter to determine if the applicant has submitted all required documents, information, fees, and other materials. The executive secretary shall notify the applicant of any materials missing from the application. The applicant has 120 days to provide the information intended to complete the application.


20:78:03:03. Incomplete application. If an applicant fails to submit required materials within 120 days of notice that materials are missing from the application, the executive secretary shall designate the application as withdrawn, or, upon receiving a written request showing good cause, may allow an applicant additional time to complete the application. The executive secretary shall notify the applicant if an application is designated as withdrawn.

             The executive secretary's designation of an application as withdrawn may be appealed to the board by written notice filed with the executive secretary within ten days after notice of withdrawal by the executive secretary. If no timely appeal to the board is filed, the executive secretary's designation of an application as withdrawn remains.


How long does the Background Check take?

The timeframe for the background investigation can vary.

  • If the first set of cards are not rejected and processed the complete report is generally received in 7-14 days from the date the cards were sent for processing.
  • If the cards are rejected, a letter and new card is mailed to the applicant/licensee to be completed. Once the second card is received it is sent for processing and if not rejected generally takes 7-14 days.
  • If the second card is rejected as well the background investigation is completed manually and can take 6-8 weeks to complete.

Under what circumstances would a file have to go before the board to be reviewed? If has to be reviewed by the board, when would the file need to be complete if needing to make a board meeting?

There are various reasons for an application file to be presented at a Board meeting. If the applicant is required to come before the Board, notification will be made during the application process. The application file would need to be completed a minimum of two (2) weeks prior to the Board meeting to be added to the agenda.

If a doctor is applying for the 60 day LT certificate and a permanent license do verifications have to be requested twice?

This would depend on if verifications are outdated by the time the full medical license application is submitted. Each license verification is time sensitive. Once the application is submitted a checklist of required items is created, you will be able to determine if the verifications need to be requested again based on this checklist.

Once the file is complete, what is the typical review time?

Once an application file is completed it will go through a final review before being issued. This process is time dependent on the number of applications also in final review.
